Free fillable General Price List template

Fill in your firm's details and prices on the left, watch a clean price list build itself on the right, then print it or save it as a PDF. It is structured around the itemized goods, services, and disclosures described in the FTC Funeral Rule, so you start from the right shape instead of a blank page.

This appears at the top of the price list. The Funeral Rule expects your name, address, and phone number on the list.

The non-declinable fee for the basic services of the funeral director and staff and overhead. Leave any price blank to print a fill-in-by-hand line.

Packaged services

Other services, facilities, and staff

The individually itemized services families can choose from.

The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

A complete price list will be provided at the funeral home.

In most areas of the country, state or local law does not require that you buy a container to surround the casket in the grave. However, many cemeteries require that you have such a container so that the grave will not sink in. Either a grave liner or a burial vault will satisfy these requirements.

If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are described at the funeral home.

This price list is provided for you to keep. Prices are effective as of the date shown above and are subject to change without notice.

An honest word on this template. This is a starting point, not legal advice, and it does not fill in any prices for you. The section headings and the disclosure wording follow the model General Price List described in the FTC Funeral Rule (16 CFR Part 453), but the Rule has specific requirements, and some states add their own. Before you use this with families, please confirm the current required items and wording against the FTC's own guide, Complying with the Funeral Rule, and your state licensing board, or have your attorney review it. Every price on your list should be your own real figure.

How to use it

1. Add your firm and prices

Type your name, address, phone, and effective date, then your price for each line. Anything you leave blank prints as a fill-in line, so you can also print a clean form and complete it by hand.

2. Check the wording

The required disclosures are written in for you. Read them against the current FTC guide and your state's rules, and adjust anything your jurisdiction words differently.

3. Print or save as PDF

Use the button to print the finished list, or choose "Save as PDF" in the print dialog to keep a digital copy. Only the price list prints, not this page around it.

4. Give families a copy to keep

The Rule expects families to be able to keep the list. Hand it over, or, better, make it easy to find online and inside a branded app so families and AI assistants can reach it too.

Common questions about the General Price List

What is a General Price List (GPL)?

A General Price List is the itemized list of the prices a funeral home or cremation service charges for its goods and services. Under the FTC Funeral Rule (16 CFR Part 453), providers must give this list to anyone who asks in person about funeral arrangements or prices, and consumers may keep it. It lets families compare prices and choose only the items they want.

What has to be on a funeral home's General Price List?

The Funeral Rule sets out specific itemized charges that must appear, including the basic services of the funeral director and staff, transfer of remains, embalming, other preparation of the body, use of facilities and staff for viewing, ceremony, and memorial services, graveside service, hearse, limousine, direct cremation, immediate burial, forwarding and receiving of remains, and prices or price ranges for caskets and outer burial containers. It must also carry the firm's name, address, and phone number, an effective date, and several required disclosures. This template lays out all of these.

Is embalming required by law?

In most cases, no. The Funeral Rule requires the price list to state that embalming is not required by law, that it may be necessary for certain arrangements such as a funeral with viewing, and that a family who does not want embalming can usually choose an arrangement that does not require paying for it, such as direct cremation or immediate burial. That disclosure is written into this template.

Can I list casket and outer burial container prices as a range?

Yes. The Funeral Rule lets a provider show a range on the General Price List as long as a complete, itemized casket or outer burial container price list is available at the funeral home. This template accepts a single price or a range for both, and includes the statement that a complete price list will be provided at the funeral home.

Does a General Price List apply to cremation providers too?

Yes. The Funeral Rule applies to providers that sell both funeral goods and funeral services, and direct cremation is one of the itemized services the list must include, along with the disclosure that families may use an alternative container. Cremation-focused providers use the same General Price List structure.

Is this template automatically FTC-compliant?

Treat it as a starting point, not legal advice. The headings and disclosure wording follow the model General Price List in the Funeral Rule, but the Rule has specific requirements, the wording can be updated, and some states add their own. Confirm the current requirements against the FTC's Complying with the Funeral Rule guide and your state licensing board, and use your own real prices, before giving the list to families.

A price list families can actually find is half the job. When your prices live on a fast website and inside your own branded app, families can reach them on their own time, and so can the AI assistants they now ask first. That is how you get found in AI, too. If you'd like to see whether your area is still open, you can check your territory in under a minute, or get in touch and we'll walk you through it at your pace.
